Brief Description
This study evaluates the implementation of the Advance Care Planning (ACP 3.0) programme in Singapore, which introduces bundled interventions to improve awareness, uptake, funding models, digital access, and expansion across healthcare and community settings. Using a mixed-methods design, perspectives from healthcare professionals, ACP clients, and nominated healthcare spokespersons will be collected through focus group discussions, structured surveys, and multi-round Delphi studies.
The evaluation aims to assess implementation experiences, adoption, and identify key ACP outcomes considered important by different stakeholders. Findings will inform recommendations to enhance ACP programme delivery and implementation nationwide.
