My Voice (Heart)

Development and evaluation of ACP-HEART: A randomized controlled trial

My Voice - Heart is a web-based intervention designed to help patients with advanced heart failure and their caregivers prepare for Advance Care Planning (ACP) by making the process simple, accessible, and digital. It supports users in reflecting on values, understanding care options, and building confidence before engaging in ACP discussions with healthcare providers.

My Voice - Heart is developed in English, Mandarin and Malay. The efficacy, reach and implementation of the intervention is being currently assessed by a RCT conducted across 5 hospitals in Singapore: National Heart Centre Singapore, Singapore General Hospital, Changi General Hospital, Sengkang General Hospital and Khoo Teck Puat Hospital.
