Evaluating the Implementation of a Palliative Care Health Coaching Program for inpatients with advanced end stage heart disease, and their caregivers

This is a project exploring the utility of palliative care health coaching for heart failure patients who were admitted currently or recently; as well as for their caregivers.

Our aims are to explore the feasibility, acceptability, and potential efficacy through a prospective cohort of sequentially recruited heart failure inpatients. Caregivers are also invited to participate if they're keen.

Outcomes include quality of life measures (KCCQ), views towards service (CSQ-4) and health care utilization.
