Bio

Clin Assoc Prof How Choon How is the Chairman, Division of Continuity Care and Senior Consultant with the Department of Care & Health Integration at Changi General Hospital (CGH). A family physician by training, he heads the population health office at CGH and works with partners at the regional health and primary care levels to improve care integration, transitions of care and care innovations in the health ecosystem to enhance care outcomes for the community.

Dr How is passionate about nurturing students from Duke-NUS Medical School, Lee Kong Chian School of Medicine, Nanyang Technological University (NTU) and Singapore University of Technology and Design (SUTD). He is also an Advanced Trauma Life Support (ATLS) Instructor, and has published regularly in peer-reviewed journals and spoken at national and international forums in the areas of Family Medicine (FM), primary care and health literacy. He has been the specialty editor for Family Medicine in Singapore Medical Journal since 2012, and his research interests are in primary care, health literacy and elderly nutrition in the community.
