Bio

Clin A/P (Dr) Tan looks after patients as their Family Physician at Pasir Ris Polyclinic since 2002. For his dedication to provide comprehensive care to his patients, he has been awarded Master Clinician by Duke NUS Medical School in 2023 and is recognised as a SingHealth Senior Clinician award in 2024. 

Clinical A/P Tan is a Fellow with the College of Family Physicians Singapore and Academy of Medicine of Singapore, who graduated as one of the six pioneer clinicians in the Master in Clinical Investigations in NUS. He has been appointed by the College as an examiner for the Fellowship candidates and for the Master in Family Medicine by the Postgraduate Medical School in National University of Singapore. 

As the Director of Research in SHP in 2012, Clin A/P Tan has developed the signature preventive health research program which was awarded the RIE 2025 Center Grant by the National Medical Research Council in 2021. He led the primary care team in the AI - Singapore Grand Challenge in 2023 to develop an AI-enabled risk counselling module targeting patients with sub optimally controlled diabetic patients, which is currently being evaluated for its effectiveness in a multi-centered randomized controlled trial. In 2026, Clin A/P Tan will lead a primary care team to develop an AI Foundation Model for primary care in collaboration with A'Star and Ministry of Health Office of Healthcare Transformation. He also partners neonatologist in Singapore general Hospital and AI team from Synapse, who have validated an AI-enabled mobile application to assess neonatal jaundice and is testing its implementation across multiple polyclinics and the tertiary hospitals.

He is passionate to share his expertise to budding primary care researchers in the department. The expanding pool of research talent has expanded the research and innovation capacity and capability in SHP and FM ACP. He also coaches Family Medicine Residents and Family Physicians in SHP and FM ACP, medical, nursing and allied health (dietetics) undergraduates from local and foreign universities in preventive health research. 
A/P Tan is the recipient of the Humanity Award in 2006 for research relating to SARS, SingHealth Excellence Award for Clinician in 2012. He is the first family physician to receive the NMRC Clinician Innovator Development Award in 2019 and has published more than 400 papers in peer-reviewed journals and received awards for oral and poster presentations at various WONCA and Family Medicine Conferences. He won the award for the best oral presentation at the 9th Asia Pacific Primary Care Research Conference in 2024.
